Looking for a CHSP - DA Service Coordination Officer

Location:  Braybrook

Reports to:  CHSP - DA Team Leader

Employment type:  Full-time

Role purpose:

The CHSP – DA Service Coordination Officer is responsible for providing a range of services and support to ensure that Domestic Assistance services are planned and implemented accordingly, under the CHSP guidelines.
The position is the key point of contact for clients, Rostering team and contractors to ensure the services are provided and maintained for the best possible outcomes, reflecting the goal directed needs of the client.

What we are looking for:

1.  Referral and Assessment Coordination:

  • Manage incoming referrals through the My Aged Care Provider portal to ensure efficient client intake.
  • Conduct assessments with clients, developing Support Plans that meet their individual needs.
  • Provide clients with relevant information, guidance, and continuous support to ensure their satisfaction and engagement.
  • Regularly contribute to the preparation and submission of data and reports to support accurate record-keeping and data analysis.

2.  Client and Service Coordination:

  • Collaborate with the Rostering team and external contractors to improve client access to necessary services.
  • Perform ongoing reviews and evaluations of services, identifying areas for improvement and implementing changes as necessary.
  • Create, maintain, and update client databases to support the accuracy and accessibility of information.
  • Act as a key point of contact for client enquiries, issues, and complaints, logging and tracking these for future reference.
  • Escalate complex client issues to management, ensuring timely and effective resolutions.

3.  Compliance and Quality Assurance:

  • Operate within the standards set out in the CHSP - DA Client Handbook.
  • Prepare programs for quality audits in coordination with CHSP - DA Team Leader to ensure compliance.
  • Assist in quality audit preparation, maintaining compliance with CHSP and organizational guidelines.

4.  Administrative Support:

  • Monitor clients’ schedules to ensure consistent and uninterrupted service delivery.
  • Review incoming invoices for services provided, verifying accuracy and addressing discrepancies.
  • Assist with report preparation, support team meetings, and contribute to efficient administrative functions within the department.
  • Other duties as required by the CHSP - DA Team Leader

Who you are:

  • Appropriate qualification in Community Services, Community Development or Aged Care, or similar discipline.
  • Demonstrated experience in providing high quality customer service
  • Experience in working with clients from diverse backgrounds
  • Relevant knowledge and experience in CHSP, HCP services or other aged care services;
  • Understanding Aged Care Quality Standards
  • Understanding of quality assurance and Occupational Health and Safety issues for self and others, particularly those at risk
  • Bilingual in Vietnamese and English
