Looking for a full-time CHSP - DA and Meals Project Officer

Reports to:  CHSP - DA and Meals Coordinator

Employment type:  Full-time

Role purpose:

The CHSP – DA and Meals Project Officer is responsible for coordinating and administering Domestic Assistance and Home Meals services in line with CHSP guidelines. The role ensures smooth program operations and high-quality, client-focused service delivery through effective communication with Clients, Contractors, and Meal Vendors. Key responsibilities include supporting client intake, monitoring satisfaction, maintaining accurate records, ensuring compliance with funding requirements, and contributing to community outreach and administrative support to meet each client’s goal-directed needs.

What we are looking for:

1. Referral Assessment and Client Intake:

  • Manage client referrals via the My Aged Care Provider Portal and support the intake process for CHSP – DA and Home Meals services.
  • Conduct assessments and develop Support Plans based on individual client needs, preferences, and funding eligibility.
  • Deliver menus, assist with meal selections, and support clients in accessing food support and other community resources.
  • Respond to client enquiries, concerns, and complaints in a timely and professional manner.

2. Service Coordination and Scheduling:

  • Coordinate the delivery of Domestic Assistance and Home Meals services in accordance with CHSP guidelines and support plans.
  • Collaborate with Rostering teams, Contractors, and Meal vendors to ensure accurate, timely, and uninterrupted service provision.
  • Prepare and manage weekly service schedules for both Domestic Assistance and Meal Delivery.
  • Provide emergency meal delivery support when required.

3. Administrative and Invoicing Management:

  • Review and verify contractor invoices for Domestic Assistance services, addressing discrepancies as needed.
  • Prepare, process, and maintain invoicing records for Home Meals services.
  • Monitor client payments and follow up on outstanding accounts.
  • Maintain accurate records of client data, documentation, and service agreements.
  • Assist with budgeting, data entry, internal reporting, and preparation for audits.

4. Communication and Stakeholder Engagement:

  • Liaise with clients, their families, contractors, and vendors to coordinate services and resolve operational matters.
  • Respond to phone and email enquiries regarding service delivery, payments, and general program information.
  • Prepare and distribute client communications, service updates, reports, and newsletters to stakeholders.

5. Program Compliance and Quality Assurance:

  • Ensure services are delivered in accordance with CHSP guidelines, the DA and Meals Client Handbook, and AVWA policies.
  • Support quality audits and maintain compliance-related documentation.
  • Participate in service reviews and quality improvement initiatives by identifying service gaps and recommending enhancements.

6. Community Engagement and Promotion:

  • Support community outreach and contribute to new client intake efforts.
  • Assist in planning and delivering marketing activities and annual client satisfaction surveys.
  • Represent the program in community events and liaise with local stakeholders to increase service visibility and access.

7. General Support:

  • Provide backup assistance to colleagues to ensure continuity of service delivery.
  • Undertake other duties as directed by management to support program success and organisational objectives.

The above list is not exhaustive and the role may change to meet the overall objectives of AVWA. Fulfil other duties as required by management and other department personnel as requested/required.

Who you are:

  • Appropriate qualification in Community Services, Community Development or Aged Care, or similar discipline.
  • Demonstrated experience in providing high quality customer service
  • Experience of working with clients from diverse backgrounds • Relevant knowledge and experience in CHSP, HCP, NDIS services or other community services.
  • Knowledge of food safety and handling regulations and dietary considerations for older people
  • Understanding Aged Care Quality Standards
  • Understanding of Quality Assurance and Occupational Health and Safety issues for self and others, particularly those at risk
