Looking for a Social Support Group Coordination Officer

Location:  Braybrook

Reports to:  PAGs Manager

Employment type:  Part-time/ Casual

Role purpose:

The Social Support Group Coordination Officer is responsible for planning, coordinating, and facilitating social support programs. This role aims to foster social connections, promote well-being, and enhance the quality of life for participants by delivering engaging group activities and events. It includes responsibilities for conducting assessments and developing personalized care plans.

What we are looking for:

A. Program Delivery

  • Plan and deliver social support group activities that are inclusive and cater to the needs of various participants.
  • Develop fortnightly/monthly schedules and organise events, outings, and social gatherings.
  • Liaise with participants to assess their interests and ensure activities align with their needs and preferences.

B. Transportation of Members

  • Transport members in line with the program's transport procedures.
  • Ensure that the Taxi Company has the correct information concerning the members using the cab and correct information recorded on the voucher and taxi form if applicable.
  • Follow the Bus Transport policies and procedures if driving the organisation’s mini buses.

C. Documentation and Compliance

  • Conduct assessments for CHSP, HACC PYP, NDIS, and HCP services.
  • Monitor and update client care plans, progress notes, and group records.
  • Ensure compliance with privacy, incident reporting, and risk management policies.
  • Manage group financials, petty cash, and administrative duties (e.g., fridge temps, fire drills, First Aid).

D. Community Networking and Team Involvement

  • Provide feedback to the Program Manager on client and team issues.
  • Support team members and participate in promotion and quality improvement activities.
  • Attend staff and program meetings, relevant training, and contribute to organisational objectives.

Who you are:

  • Experience caring for aged persons, people with disabilities, and their carers.
  • Minimum 2 years in Social Support Groups, Lifestyle Programs, or similar roles
  • Experience with diverse groups, including seniors and culturally diverse communities.
  • Knowledge of local services and networks.
  • Ability to plan and run activities tailored to participants’ needs.
  • Ability/ Willing to drive a 12-seater minibus and handle challenging behavior.
  • Understanding of aging and dementia.
  • Team player, self-motivated, creative, and flexible.
  • Willing to work weekends if needed.
  • The ability to speak a second language, especially East Timorese, Maltese, or Vietnamese, would be highly advantageous.
