Social support – individual is assistance provided by a companion (paid worker or volunteer) to frail and isolated elderly clients, either within the home environment or while accessing community services, which is primarily directed towards meeting the person’s need for social contact and/or company in order to participate in community life.

What are activities included?

 Visiting services

 Telephone and web-based monitoring services (including other technologies that help connect older people to their community e.g. to assist people with sensory impairments or those living in geographically isolated areas)

 Accompanied activities (such as assisting the person through accompanied shopping, bill paying, attendance at appointments and other related activities).

Eligibility

If you are over 65 please contact My Aged Care on 1800 200 422 or access www.myagedcare.gov.au for self-referral. 

Remember to let them know that you want the Australian Vietnamese Women’s Association to be your preferred service provider.

Social Support Group Services

Social support – Groups (formerly known as Planned Activity Groups), which are operated weekly, provide opportunities for clients to attend and participate in social interactions, which are conducted at a centre, online, or through group outings. These activities focus on participants’ needs and aspirations in order to enable them to improve their health and well-being. The needs may be physical, emotional, intellectual, recreational, cultural, linguistic, and spiritual.
