How to get started

 

 Step 1 

 

Call us

We'll discuss your situation, answer your questions,  understand your eligibility, and what you need. Then we will help you to register with My Aged Care, explain the next steps and what to expect.

 Step 2 

 

My Aged Care assessment 

 

You need an assessment to receive funded services. We'll guide you through getting referred, help you prepare relevant information and documents that may be required for the assessment. After your assessment, we'll follow up with you, help you understand your support plan when it arrives, and explain your funding level and service options.

 Step 3 

 

 Start Support at Home services

 

Once your funding is approved and available, call us so we can get started. We'll match you with a care partner who will visit you at home to create your personalized care plan together. You'll discuss your routines, preferences, and goals, and your care begins from there.  

Switching from another provider?  

We’re here to make your transition simple and stress-free. Our team will coordinate with you, ensuring your services continue smoothly and avoiding interruption to your care and support.

About the Support at Home program

 

 From 1 November 2025, the Australian Government introduces a new program called Support at Home.

It replaces Home Care Package and Short term Restorative Care Programs with a single, simpler system for in-home aged care. 

What this means for you: 

  • If you’re already receiving care, it will continue.  
  • Your current services and funding remain in place while the new program is rolled out.  
  • The new system introduces eight support classifications, plus Restorative and End-of-Life pathways, and dedicated Assistive Technology & Home Modifications.

Short-term support programs

 
Support at Home also includes specialised short-term help for specific situations:
 

 

 

Restorative Care

Help to regain your strength and independence after illness, injury, or a hospital stay. You'll work with physiotherapists, occupational therapists, and other specialists for up to 12 weeks. The goal is to help you recover and get back to your routine.

Assistive Technology & Home Modifications 

 

Access to equipment like mobility aids, shower chairs, or bed rails and home modifications like ramps or grab rails through separate funding—so you can get what you need without using your regular care budget.

End-of-Life Care

 If you've been told you have three months or less to live, additional support is available to help you stay at home comfortably for as long as possible. Up to $25,000 in funding covers compassionate, dignity-focused care for up to 16 weeks. We coordinate everything so you and your family can focus on being together.
